Job Description
We are seeking a motivated and compassionate Clinical Liaison (LVN) to join our team. This role is perfect for a licensed vocational nurse who thrives on relationship-building, care coordination, and compliance oversight. The Clinical Liaison serves as the vital bridge between clients, families, referral partners, and our care team—developing customized care plans that reflect each client’s unique history, health status, and lifestyle.
Key Responsibilities
Build and maintain referral relationships with hospitals, clinics, physicians, and community organizations.
Facilitate client referrals and ensure a smooth onboarding process.
Perform in-home assessments to evaluate medical, functional, and personal care needs.
Create and oversee individualized care plans tailored to client preferences and needs.
Provide in-service training and ongoing education to the caregiving team to support quality standards.
Ensure compliance with state regulations, company policies, and best-practice clinical standards.
Maintain thorough and accurate documentation of assessments and care coordination.
Caregiver in-service trainer
Act as a trusted support for clients and families, offering education and guidance.
Qualifications:
Current LVN license in Texas, active and in good standing. (Preferred)
Minimum 2 years of nursing or healthcare experience (home health, private duty, or care management preferred).
Strong knowledge of compliance and regulatory guidelines in home care.
Excellent communication, outreach, and relationship-building skills.
Ability to work independently while balancing multiple priorities.
Compassionate, client-centered approach to care.
